Does anyone have a form they use to to release students names and photos for media coverage? Or a form they use to post student information (name & photo) on the school web site?
I used to use the form I attached. However, stopped using it when I realized that since they sign a school form, I am actually covered. Once all the school forms are in, I e-mail the secretary who has entered all the students in a database that have turned in their forms. If I get new students in, I still revert to this form from time to time.
I also include this note on the class webpage:
"The pictures I take during class get compiled into short videos about agriculture class projects. Your picture will not appear in a video unless you have turned a form into school in September granting these permissions. If I have accidentally included a picture of you, please notify me immediately and I will remove the video. Thank you!"
